Two Injury Crashes Reported in Apple Valley Wednesday Evening; Multiple Drivers Transported, Citations Issued

APPLE VALLEY, MINNESOTA—Apple Valley Police, Fire, and Allina EMS responded to two separate injury crashes just over an hour apart on Wednesday, November 19, 2025. One on Cedar Avenue involving four vehicles, and a second collision at the intersection of Galaxie Avenue and Fordham Avenue involving two vehicles. Both incidents resulted in EMS transports and traffic delays as crews worked to clear the scenes.

Crash #1: Four-Vehicle Rear-End Collision on Cedar Avenue

A chain-reaction crash involving four vehicles occurred around 5:50 p.m. in the southbound lanes of Cedar Avenue at 145th Street West, according to the crash report provided by the Apple Valley Police Department.

According to the crash report, the crash occurred when traffic ahead had slowed when the driver of Vehicle 1, a 32-year-old man from Lakeville, failed to stop in time and struck the rear of Vehicle 2, a Honda HR-V driven by a 17-year-old from Minneapolis. The impact pushed Vehicle 2 into Vehicle 3, which was then forced into Vehicle 4.

The driver of Vehicle 1 told officers he had been distracted by looking out the window and did not notice the traffic stop. He was cited for failure to use due care. The crash report lists the primary contributing factor of the crash as driver inattention, as roads were reported to be clear and dry, despite it being dark with streetlights on. It was determined that speed was not considered a factor. The driver of Vehicle 3 was also cited after admitting they did not have insurance.

The 17-year-old driver of Vehicle 2 suffered suspected serious injuries, according to the crash report, and was transported by Allina EMS to a local hospital. All other drivers and passengers reported no apparent injuries.

Damage summary:

  • Vehicle 1: Significant front-end damage; towed

  • Vehicle 2: Moderate damage; towed

  • Vehicle 3: Moderate rear-end damage; towed; driver cited for no insurance

  • Vehicle 4: Minor damage; driven from the scene

Crash #2: Two-Vehicle Intersection Collision at Galaxie Avenue & Fordham Avenue

Just over an hour after the Cedar Avenue incident, around 7:05 PM, Apple Valley Police, Fire, and Allina Health responded to a second crash at Galaxie Avenue and Fordham Avenue involving a teen driver operating on an instruction permit and a northbound SUV.

According to the crash report, Vehicle 1, a Chrysler 300 driven by a 19-year-old Rosemount woman, attempted to turn from westbound Fordham Avenue onto northbound Galaxie Avenue. At the same time, Vehicle 2, driven by a 42-year-old Shakopee woman, was traveling straight through the intersection on northbound Galaxie. According to the crash report, Driver 1 told officers she believed Vehicle 2 was preparing to turn because she saw a right-turn signal activated. Thinking the SUV was about to turn onto Fordham Avenue, she attempted her turn and pulled directly into the path of Vehicle 2. Vehicle 2’s driver was unable to provide a full statement due to pain, but her passenger confirmed that Vehicle 1 pulled out unexpectedly, leaving Vehicle 2 no time to stop, resulting in the collision at the T-intersection. Following the collision, the vehicles came to rest off the road on the southbound side of Galaxie Avenue just south of the entrance to Galaxie Park. Southbound Galaxie Avenue had to be closed at Upper 136th Street West while crews investigated and cleared the crash.

Allina EMS transported all involved parties to M Health Fairview Ridges Hospital for the following injuries:

  • Driver 1 (Vehicle 1): Suspected minor injury

  • Driver 2 (Vehicle 2): Suspected minor injury

  • Passenger in Vehicle 2: Possible minor injury

All occupants were wearing seat belts; airbags deployed in both vehicles. Both vehicles sustained disabling damage and were towed.

Officers determined that failure to yield the right-of-way caused the crash. Driver 1 was cited for Instruction Permit Violation and Failure to Yield to Right-of-Way. The conditions at the time of the crash were once again clear and dry, with dark conditions with streetlights on.
