Driver Killed in Four-Vehicle Crash at Apple Valley Intersection; Investigation Confirms Red Light Violation

Published: April 3, 2026

APPLE VALLEY, MN: New details have been released following a serious four-vehicle crash last Thursday afternoon at a busy Apple Valley intersection that left one person dead. According to a Minnesota Motor Vehicle Crash Report from the Apple Valley Police Department, the crash occurred at approximately 3:20 p.m. on April 2, 2026, at the intersection of Pilot Knob Road and 157th Street West.

Authorities confirmed that four vehicles were involved in the collision. The crash occurred in daylight with wet road surfaces, and the traffic signals at the intersection were operational at the time.

Investigators determined that a 2006 Ford Focus (Vehicle 1) was traveling southbound on Pilot Knob Road, a 2024 Chevrolet Silverado (Vehicle 2) was traveling westbound on 157th Street West, and two additional vehicles, both stopped at a red light eastbound on 157th Street, were also struck. According to the crash report narrative, the Ford Focus ran a red light before entering the intersection.

The report details a chain-reaction collision. According to the report, the Ford Focus entered the intersection against a red light and struck the passenger side of the Chevrolet Silverado. The Ford Focus then continued forward, hitting a stopped Hyundai Tucson, and also struck the rear driver’s side of a Hyundai Palisade.

Investigators noted that the Ford Focus sustained severe, disabling damage to both the front and roof, indicating a high-impact collision.

Fatality Confirmed, One Additional Minor Injury Reported

The crash report confirms that the driver and sole occupant of the Ford Focus, a 68-year-old woman from Apple Valley, was killed in the crash.

Emergency crews had previously extricated the driver from the vehicle before transporting her. The report indicates she later died as a result of her injuries.

Other individuals involved in the crash reported minimal or no injuries:

  • The driver of the Chevrolet Silverado sustained a minor hand injury but declined transport

  • Drivers of the other two vehicles reported no injuries

  • A juvenile passenger in one of the vehicles was also not injured

All individuals were reported to be wearing seatbelts.

Witness Statements & Investigation

Multiple witnesses confirmed to investigators that the Ford Focus entered the intersection on a red light, while cross traffic had a green signal.

The Minnesota State Patrol’s reconstruction team responded to the scene and completed a full crash reconstruction. All vehicles were towed for further examination.

The crash report lists “ran red light” as the primary contributing factor, with no indication of alcohol or drug involvement.

Road Closures and Emergency Response

As previously reported, the crash caused significant disruptions during the afternoon commute:

  • Southbound Pilot Knob Road was closed at 155th Street

  • Eastbound 157th Street was closed west of the intersection

Apple Valley Police, Apple Valley Fire, Allina Health EMS, and the Minnesota State Patrol all responded to the scene.

Summary

This updated report confirms that what was initially believed to be a serious injury crash has now been classified as a fatal crash, with investigators determining that the incident began when a vehicle entered the intersection against a red light, triggering a multi-vehicle collision. The deceased has not been identified.
