West St. Paul Woman Charged With Felony Forgery for Allegedly Altering Nursing Home Medication Records

WEST SAINT PAUL: A West St. Paul woman is facing a felony charge after authorities allege she admitted to altering a nursing home's medication records to conceal the removal of prescription pain medication intended for a patient who had previously died .
According to a criminal complaint filed in Dakota County, Debra Lynne Stone, 62, has been charged with one count of forgery involving a record or document of a private business, a felony under Minnesota law.
According to the complaint, on March 19, 2026, an officer with the West St. Paul Police Department responded to a report of a controlled substance theft at a nursing home in Dakota County.
The facility's nursing director told police that Stone had allegedly removed six Hydromorphone tablets that had been prescribed to a patient who had died. The complaint states staff discovered the patient's prescription log had been altered, changing the remaining pill count from 23 to 17. Authorities allege the final three entries on the log had been written over, and another nurse reported the entries had been altered because she had been the last person to document the medication count before the patient's death.
According to the complaint, the nursing director reported that Stone admitted to taking the six Hydromorphone tablets and altering the medication log to match the reduced count.
Police later interviewed Stone at her residence, where she allegedly admitted to both taking the medication and altering the prescription log, according to the complaint. Stone reportedly told officers she took the pills because her husband was in pain but later flushed them down the toilet because she knew she should not have taken them.
The allegations contained in the criminal complaint are accusations. Stone is presumed innocent unless and until proven guilty in court.
